Hemorrhoids / Piles

Hemorrhoids, also called piles, are vascular structures in the anal canal. In their normal state, they are cushions that help with stool control. They become a disease when swollen or inflamed; the unqualified term "hemorrhoid" is often used to refer to the disease. The signs and symptoms of hemorrhoids depend on the type present. Internal hemorrhoids often result in painless, bright red rectal bleeding when defecating. External hemorrhoids often result in pain and swelling in the area of the anus. If bleeding occurs it is usually darker. Symptoms frequently get better after a few days. skin tag may remain after the healing of an external hemorrhoid.
While the exact cause of hemorrhoids remains unknown, a number of factors which increase pressure in the abdomen are believed to be involved. This may include constipationdiarrhea and sitting on the toilet for a long time. Hemorrhoids are also more common during pregnancy. Diagnosis is made by looking at the area. Many people incorrectly refer to any symptom occurring around the anal area as "hemorrhoids" and serious causes of the symptoms should be ruled out.Colonoscopy or sigmoidoscopy is reasonable to confirm the diagnosis and rule out more serious causes.
Often, no specific treatment is needed.Initial measures consist of increasing fiber intake, drinking fluids to maintain hydration, NSAIDs to help with pain, and rest. Medicated creams may be applied to the area, but their effectiveness is poorly supported by evidence.A number of minor procedures may be performed if symptoms are severe or do not improve with conservative management.Surgery is reserved for those who fail to improve following these measures.
Approximately 50% to 66% of people have problems with hemorrhoids at some point in their lives.Males and females are both affected with about equal frequency. Hemorrhoids affect people most often between 45 and 65 years of age.It is more common among the wealthy. Outcomes are usually good.The first known mention of the disease is from a 1700 BC Egyptian papyrus.

Piles/ INTERNAL & EXTERNAL hemorrhoids… are swellen & inflamed vascular structure of anal canal.
The common symptoms of INTERNAL hemorrhoid is bright red RECTAL bleeding during defecating & INTERNAL hemorrhoid could be classified as grade 1 to grade 4as per size & severity of hemorrhoidal mass.
The painful swelling in anal canal is common feature of EXTERNAL hemorrhoids,


Treatment:…
MCDPA… is the first line & key THERAPY.

Sclerotherapy:..... sclerosing agent produces contraction of veins & for this purpose on hemorrhoidal veins sclerosing agent like aqueous solution of kshar of different concentration are used in form of submucosal injection (superficial) in the pile mass.
The procedure takes about 5minutes & patient feels only a pricking or pinching sensation so it's a painless & ambulatory procedure. patient can go home after 5 to 10 minutes of the procedure.
The MCDPA is followed up to next 15 20 days.


PRATISARANIYA kshar karma… kshar paste of high concentration is applied on 1st&2nd grade hemorrhoids which causes fibrosis of pile mass within 1 to 2 minutes.the whole procedure takes about 5 to 10 minutes & patient can do his daily routine works & go home after 30 minutes.


The MCDPA is followed up to next 15 to 30 days.

R.B.L. TECHNIQUE … A medicated band is applied at the base (root) of hemorrhoids which stops the blood supply of pile mass & within 5 to7 days the pile mass becomes ischaemic & necrosed leading to falling of hemorrhoid.
In severe cases RBL could be used with sclerotherapy, PRATISARANIYA kshar Karma or kshar sutra  ligation as per need for better & faster recovery.
This nearly painless procedure takes about 10 to 15 minutes & patient can do his routine work after 30 minutes of treatment.
The MCDPA is followed up to next 15 to 30 days.

Kshar sutra ligation treatment (KSLT)… IN 2nd,3rd&4th grade hemorrhoids & in EXTERNAL hemorrhoids kslt is used to ligate the base(root) of pile mass which causes necrosis & ischemia by its caustic property & stops bleeding.
This procedure takes about 10 to 15 minutes and patient can go home after 30 minutes of treatment.
The kslt can be used with PRATISARANIYA kshar karma, sclerotherapy or RBL technique as per severity of diseases.
The MCDPA is followed up to next 15 to 30 days for better and faster recovery.

Agni karma (thermal cauterization)… is used in chronic hemorrhoids which protrudes anal orifice, larger in size,has rough irregular surface, became hard due to chronic inflammation & fibrosis with time.
The procedure takes about 15 20 minutes & patient can go home after 3 to 6 hours of treatment & can start his normal routine works after 1 or 2 days.
